This condition often appears concurrently with other menstrual disorders and should be noted in clinical practice, not viewed in isolation.Menstruation with clots is commonly caused by etiologies such as qi stagnation, cold congelation, qi deficiency, and heat scorching.
(1) Qi stagnation: Qi governs blood, and when qi flows, blood flows. When qi stagnates, blood becomes congealed. Emotional distress, liver qi stagnation, and impaired qi movement can lead to obstructed menstruation and the formation of clots.
(2) Cold congelation: "When the temperature is warm, blood flows smoothly; when the temperature is cold, blood congeals," and "blood congeals into clots when exposed to cold." External cold pathogens, internal cold injury, or a naturally deficient yang constitution with internal cold can cause cold to accumulate in the (chong-ren) channels, leading to blood congealing and the formation of clots during menstruation.
(3) Blood heat: External heat pathogens, excessive consumption of pungent and hot foods, overuse of warming uterine herbs, or emotional distress that transforms into internal heat can cause heat to accumulate in the channels. Blood is then scalded by heat, resulting in the formation of clots during menstruation.
(4) Qi deficiency: Excessive fatigue or prolonged illness can deplete vital qi, weakening its ability to move blood. This causes delayed blood flow and the formation of stagnant clots during menstruation.
