What's wrong with lower abdominal pain when menstruation doesn't come?

Patient's question:

10 days of menstruation, this month hasn't arrived yet. The past two days, I occasionally have a dull lower abdominal pain and a bloated feeling in my stomach. My breasts are also swollen! My external genital area is slightly itchy.
Medical history and treatment effectiveness: On June 5th, I had a cervical erosion surgery, and the recovery was very good! The Ureaplasma urealyticum infection has been treated with two courses of medication. I haven't gone for a follow-up yet.
Help needed: Could you please tell me what might be causing this lower abdominal pain? What should I do?

Doctor's answer:

This is a symptom of dysmenorrhea, which can be regulated. Dysmenorrhea can be divided into two types: primary and secondary. Primary dysmenorrhea refers to dysmenorrhea without any organic lesions in the reproductive organs, commonly occurring within 6 to 12 months after menarche or during the initial establishment of the ovulatory cycle. Secondary dysmenorrhea refers to dysmenorrhea caused by organic lesions in pelvic organs, such as endometriosis, pelvic inflammatory disease, or cervical stenosis.
Currently, it is believed that increased levels of prostaglandins in the endometrium and blood are the main cause of dysmenorrhea. Because a large amount of prostaglandins can stimulate the uterus, causing strong contractions of uterine muscles, leading to uterine ischemia and hypoxia, which produce severe pain, resulting in dysmenorrhea. Similarly, a large amount of prostaglandins can also cause muscle contractions in the gastrointestinal tract, so women with dysmenorrhea may also experience gastrointestinal symptoms such as nausea, vomiting, and abdominal pain.
