How to relieve bleeding after inserting an IUD during sexual intercourse

Patient's question:

I had the IUD for two months. At the one-month check-up, it was confirmed that the IUD was still in the uterine cavity. Intercourse would cause mild tearing, not severe tearing that happens every time we have sex. The bleeding has a slightly fishy smell. Without intercourse, there would be absolutely no bleeding. After this tearing, my labia minora have been a bit itchy. Over these two months of having the IUD, I haven’t experienced any discomfort except for bleeding during intercourse. I haven’t given birth before. Should the IUD be removed?

Doctor's answer:

Bleeding during intercourse, only occurs during intense activity. This needs to rule out inflammation of the external genitalia, vagina, and cervix. A fishy odor indicates inflammation. First, rule out the inflamed areas, treat the inflammation, and then decide whether to remove the IUD.
