Patient's question:
For 8 consecutive months, the menstrual cycle is regular but advances by 4 days, with reduced flow. Spleen and stomach Qi deficiency, kidney Yang deficiency.Doctor's answer:
The reasons for reduced menstrual volume can be attributed to three possible factors:1. Excessive levels of follicle-stimulating hormone (FSH) and luteinizing hormone (LH) in the blood, leading to a feedback inhibition of the ovaries in producing estradiol and progesterone. This condition is known as premature ovarian failure. It initially manifests as a decrease in menstrual volume, followed by a delay of 1–2 months in the menstrual cycle, and ultimately results in amenorrhea.
2. Elevated testosterone (androgen) levels in the blood are commonly associated with polycystic ovary syndrome (PCOS). Increased testosterone can neutralize estradiol in the blood, reducing estradiol levels and affecting follicular development and ovulation. Additionally, excessive testosterone can impede the normal development of follicles, promoting their atresia and closure.
3. Elevated levels of prolactin (PRL) in the blood can suppress ovarian function. This may manifest as infrequent menstruation, reduced menstrual volume, anovulation, and luteal phase deficiency, ultimately leading to amenorrhea.
