Can you get pregnant without ejaculation or climax?

Patient's question:

I had sex with my girlfriend multiple times, but I did not ejaculate or reach orgasm, while she had a few orgasms. Will this result in pregnancy?

Doctor's answer:

Hello: If there is no ejaculation, there should be no possibility of pregnancy.
Impotence without ejaculation is a type of male sexual dysfunction. It is characterized by some patients who can achieve erection without difficulty during sexual activity, and the erection is very firm, lasting a long time without fatigue, but they cannot reach climax or ejaculate, let alone pleasure. In actual life, such patients are not uncommon, but many are too embarrassed to speak up and avoid seeking medical help, often silently enduring the distress caused by sexual issues and infertility. Since this condition primarily affects young and middle-aged adults, improper handling can affect marital relationships and even lead to family breakdown, causing mental distress to the patients. Functional impotence is the most common type of ejaculation disorder, accounting for more than 90% of such cases. Here, we will provide a brief introduction to what constitutes functional impotence.
Functional impotence includes the following three aspects:
1. Central impotence: Primarily caused by abnormal brain function, leading to increased inhibitory effects on sexual arousal, especially on the ejaculation center, resulting in patients lacking sexual desire climax and ejaculation. In some cases, it is also caused by sensory organ disorders, emotional factors, or intellectual impairments.
2. Spinal impotence: Refers to dysfunction or failure of the ejaculation and erection centers in the lumbar and sacral spinal cord, which can lead to weakened or complete inability to achieve erection, as well as delayed or complete inability to ejaculate.
3. Psychological impotence: This type of impotence does not involve organic abnormalities in the reproductive organs.
