Will an ultrasound not finding a gestational sac indicate an ectopic pregnancy?

Patient's question:

I had my period on February 10th, and I was with my boyfriend on February 25th. I didn't get my period on March 10th. On March 22nd, I took a pregnancy test and it showed two lines. On March 24th, I took another test and it also showed two lines. On March 25th, I had a thorough check-up. The hCG level was 58.1, and the progesterone level was 38.63, indicating the follicular phase; 0-0.87, and the luteal phase; 0.37-18.4. The ultrasound examination showed that the bladder was well-filled, the walls were smooth, and the internal echo was uniform. The uterus was anteriorly positioned, measuring 533047mm with no echoes, and the thickness was about 4mm. A 2624mm anechoic area was detected in the right adnexal region. No abnormal echoes were observed in the left adnexal region. CDFI showed no obvious abnormal blood flow signals. Could it be an ectopic pregnancy if the ultrasound couldn't find a gestational sac?

Doctor's answer:

An ultrasound may not be able to detect a gestational sac, and your hCG levels may not be considered high. I suggest you get another ultrasound at 40 days. Please do not attempt to self-diagnose an ectopic pregnancy. It is completely possible for an ultrasound to not show anything within the first month. Wait until 40 days to check again, and it must be confirmed whether the gestational sac is in the uterine cavity or another location. Patients face the risk of ectopic pregnancy, so we should actively conduct examinations and treatments without avoiding medical visits. If the ectopic pregnancy is not treated effectively, the treatment plan should be adjusted promptly.
