Patient's question:
Female, 29 years old, has had three induced abortions and about ten medication abortions over the past ten years. She got divorced in 2016 and naturally miscarried in May 2017 after being pregnant for over a month. Later, an ultrasound revealed inflammation with cervical cysts and endometrial thickness of 15mm. Progesterone, human chorionic gonadotropin, and estriol levels were all low, but the inflammation did not fully heal on its own. In March of this year, she became pregnant again but miscarried again after a month. If she needs a comprehensive pre-pregnancy checkup, what tests should be included? If any are not necessary, please do not recommend them.
Doctor's answer:
You currently have two cases of adverse early pregnancy. In this situation, it is recommended that both partners undergo thorough examinations. First, both male and female partners should undergo karyotype testing. The male partner needs to have a 3-7 day semen routine test, including a sperm morphology test, sperm DNA damage test, and other related examinations.2. The female partner should undergo a complete TORCH test, thyroid hormone test, and anti-phospholipid antibody test to rule out conditions such as intrauterine adhesions.
3. It is recommended to temporarily continue breastfeeding. Only after the test results are normal should you attempt to conceive, paying particular attention to the anti-phospholipid antibody test.
4. Maintain a positive mindset, avoid overwork, and engage in regular physical activity.
