Is ectopic pregnancy a serious possibility?

Patient's question:

I had my last menstrual period on February 25th of this year, and it has been 37 days since then. I went to the hospital for blood HCG and found it to be 4687, with progesterone at 22.51. The follicular phase was 0.4-1.1, the ovulation phase was 0.5-0.75, and the luteal phase was 3.6-21.6. I also had an ultrasound, which showed no gestational sac in the uterus, with a membrane thickness of 0.6. The left ovary measured 2.8×1.6, and the right ovary measured 2.9×1.8. There were no abnormal blood flow signals in the uterine adnexa. The day before yesterday, I traveled back from a distant place by car for 8 hours. Now, I have some brownish (discharge) downstairs. Is it because the gestational sac is too small to be seen, or could it be an ectopic pregnancy? I am desperate for help. Thank you!

Doctor's answer:

According to the results of the urine test, it can be confirmed that you are pregnant. Currently, no gestational sac is observed inside the uterus, which is due to the short duration of the missed period. Therefore, the minimal amount of brownish discharge now may be a sign of miscarriage caused by excessive fatigue from a long journey. However, it is still not possible to completely rule out the possibility of ectopic pregnancy.
1. You can first undergo treatment to support the pregnancy for 1-2 weeks to see how it goes.
2. Rest in bed absolutely, avoid breastfeeding activities and physical exertion.
There are many treatment methods for ectopic pregnancy, but since the patient's condition varies, the chosen treatment method will also differ. Therefore, patients need to undergo timely and thorough examination and diagnosis to receive symptomatic treatment.
