Bleeding - Is it a miscarriage?

Patient's question:

I just bought a pregnancy test kit two days ago and found out I'm three weeks pregnant. But suddenly yesterday, I had a little bleeding, just a tiny amount, and didn't feel anything else in my body. Today, there's a bit more blood and some abdominal pain. I want to ask how to have a miscarriage if there's bleeding.

Doctor's answer:

Because you are currently only three weeks pregnant, at this stage, it is normal for the gestational sac not to reappear. If there is bleeding at this time, there are two possibilities. One is that it may have constituted a fertilized egg, but the implantation was successful, and the bleeding now is a normal menstrual bleeding. This condition is called a biochemical pregnancy. The other possibility is that the gestational sac reappears, indicating threatened miscarriage.
