What are the reasons for sperm always flowing out?

Patient's question:

It is the transport tool for sperm to be delivered from the male reproductive tract to the female reproductive tract, and also a buffering substance. Why is sperm always flowing out?

Doctor's answer:

Some infertile women often complain during their visits that most of the semen flows out after intercourse and worry that this is the reason for their infertility. In fact, semen leakage after intercourse is a normal phenomenon. Semen consists of sperm and seminal fluid, which is composed of secretions from the epididymis, seminal vesicles, prostate, and bulbourethral glands and their adjacent glands. After ejaculation, semen is stored in the posterior fornix of the female vagina, and the cervical mucus secreted by the cervix acts as a barrier. Only healthy and motile sperm can move quickly toward the cervix through the tail's movement and pass through the cervical mucus with the help of uterine contractions and fallopian tube peristalsis, through the uterine cavity, and finally reach the fertilization site—the ampulla of the fallopian tube. However, abnormal, low-motile, dead sperm, and other components of the semen cannot pass through the cervix and enter the uterine cavity.
