How should cerebral edema be prevented?

Patient's question:

How should cerebral edema be prevented?

Doctor's answer:

Newborns should be cautious or use minimal amounts of mannitol to avoid sudden intracranial pressure drops that could exacerbate intracranial hemorrhage, or sudden increases in blood volume that could lead to heart failure and other complications. In contrast, the triple therapy of furosemide combined with albumin and dexamethasone is effective in treating neonatal intracranial hypertension, with a slower and more sustained action, without the side effects associated with mannitol. However, for certain critically ill patients, such as those with respiratory failure that threatens their lives, a small dose of mannitol (0.25 g/kg) can be administered slowly intravenously on the basis of the above treatment. Dexamethasone should be used in high doses (0.5-1 mg) every 4-6 hours until symptoms improve, after which the dose should be reduced.
