Patient's question:
How is allergic purpura treated in the early stage?Doctor's answer:
There is hematuria. Do you have any discomfort symptoms? What are your allergies? How is the effect of the medication?Treatment for allergic purpura:
1. Remove the cause: It is important to identify and eliminate allergens.
2. General treatment:
(1) Antiallergic drugs: Chlorpheniramine 4mg, three times daily orally; Diphenhydramine or Hydroxyzine 25mg, three times daily orally; Loratadine 10mg, once daily orally; 10% calcium gluconate 0ml, intravenous injection, once daily.
(2) Vitamin C: It can enhance capillary resistance. The dosage of vitamin C should generally be high. Intravenous injection is preferred for vitamin C. Rutin 20-40mg, twice daily orally; Vitamin C 2-3g, once daily intravenous injection or added to glucose solution for intravenous infusion.
(3) Hemostatic drugs: Tranexamic acid; Amicar.
3. Corticosteroids: Prednisone 30-40mg, once daily orally. In severe cases, hydrocortisone 100-200mg or dexamethasone 10-20mg can be used for intravenous infusion daily, for 3-5 days, then switch to oral administration after improvement.
4. Immunosuppressants: For nephritis or membranous/proliferative nephritis complications where single-hormone therapy is ineffective, cyclophosphamide 2-3mg/kg/d intravenous injection or azathioprine 2-3mg/kg/d orally can be used. However, blood counts and other side effects should be monitored. Dipyridamole may also reduce proteinuria.
