What is IgA Nephropathy with Crescentic Glomerulonephritis

Patient's question:

Hemolytic uremic syndrome with crescentic glomerulonephritis

Doctor's answer:

Performance and Diagnosis
1. International Society of Kidney Diseases in Children (ISKDC) Pathological Classification
Grade I: Minimal change disease.
Grade II: Mesangial hyperplasia.
Grade III: (a) Focal and (b) diffuse hyperplasia or sclerosis, with crescent formation in 75%.
Grade VI: Membranoproliferative lesions.
2. World Health Organization (WHO) Pathological Classification
Grade I: Includes minimal change disease, minimal change disease with focal segmental glomerulosclerosis, and mild focal proliferative glomerulonephritis.
Grade II: Includes mild diffuse proliferative glomerulonephritis and mild diffuse proliferative glomerulonephritis with focal segmental glomerulosclerosis.
Grade III: Includes moderate focal proliferative glomerulonephritis and moderate diffuse proliferative glomerulonephritis.
Grade IV: Includes severe diffuse proliferative glomerulonephritis and end-stage renal disease.
Guidelines:
Traditional Chinese Medicine (TCM) and Western medicine each have their strengths in treating this condition. However, Western medicine lacks particularly effective methods for treating various types of nephritis, renal failure, and uremia. Western hormone-based medications have limited efficacy, significant side effects, and are not ideal, with high costs. The condition tends to recur, as these treatments only address symptoms rather than the root cause, causing patients suffering and potentially leading to numerous complications in the long term. Dialysis, used to treat uremia and renal failure, can lead to gradual atrophy and failure of the kidneys and other organs over time. Ultimately, kidney transplantation may be required, but its survival period is also limited.
Prevention:
First, avoid exposure to various "unhealthy influences" that may trigger this condition. Avoid dietary triggers such as fish, shrimp, crabs, pollen, and milk, which may induce allergic purpura nephritis. Second, pay attention to keeping warm, preventing colds, exercising regularly, and strengthening the body to enhance disease resistance. Third, after diagnosis, rest in bed, avoid overexertion, moderate sexual activity, and avoid smoking and alcohol. The diet should be rich in nutrients and easy to digest, with plenty of fresh vegetables and fruits. For patients with hematuria, avoid spicy, dry, irritating foods and seafood, such as rooster, sea fish, beef, lamb, and geese, to prevent exacerbating the condition by promoting heat and fire. Patients with excessive proteinuria should avoid excessive high-protein diets to prevent malabsorption.
