What is the reason for low post-meal blood sugar after taking insulin?

Patient's question:

Previously tested as Type 1, taking NovoRapid 5.0R in the morning at 12 and in the evening at 8. Before and after breakfast and dinner, blood sugar levels are around 5-6, with slightly low post-meal blood sugar and almost normal at noon.

Doctor's answer:

Postprandial hypoglycemia is due to a high dose of pre-meal short-acting insulin. Switch to NovoRapid 30R and measure postprandial blood glucose. Adjust by ±1 unit based on postprandial blood glucose values until it returns to normal. Regularly monitor blood glucose, blood lipids, blood pressure, and urine microalbumin. If blood pressure is high, use losartan to protect the heart, lower blood pressure, and reduce protein levels. Follow a low-sodium, low-protein diet. Consult an ophthalmologist for cataract treatment.
