Hematochezia without pain could it be hemorrhoids?

Patient's question:

Recently, I've been eating a lot more over the past couple of days. I've had two bowel movements, and both were not smooth. Both times I've had blood in my urine, and it was mixed with blood. At first, I thought it was just a stomach issue, but it doesn't hurt much. Could it be hemorrhoids if the bowel movement has blood but no pain?

Doctor's answer:

Not all cases of blood in the stool must be accompanied by pain, nor does all urinary bleeding from hemorrhoids always cause pain. In some cases, internal hemorrhoids may bleed without damaging the local nerves, especially when the rectal mucosa above the dentate line is affected or when hemorrhoids rupture acutely, as the nerves in these areas lack pain sensation. This can lead to multiple episodes of painless bleeding or erosion. In such situations, it is first necessary to perform a thorough examination with an anoscope to determine whether the cause of the bleeding is due to internal hemorrhoids, mucosal erosion, or inflammation-induced breakdown.
