Patient's question:
Patient is 41 years old, began experiencing tinnitus and hearing loss eight months ago. After treatment, dizziness was suppressed, but hearing slightly improved. Currently experiencing dizziness again and hearing impairment. No surgery is planned. What are the characteristics of otosclerosis?Doctor's answer:
1, Etiology unclear. Progressive hearing loss accompanied by dizziness. Often recurs during puberty, initially affecting one side, with remission during childhood. May include Webers misalignment, mild vertigo. Family history is common.2, Examination: Tympanic membrane is thin or normal, may show Schwartze sign (red reflex), and Eustachian tube is patent.
3, Early stage is conductive hearing loss, with hearing curves rising from low frequencies, mid-stage curves are flat, bone conduction curves show Carhart notch (Y-notch), late stage is mixed hearing loss. Gelle's test is negative.
4, Impedance audiometry shows reduced compliance, tympanogram is Type As, stapedial reflex is absent.
5, Mastoid X-ray shows good pneumatization, multi-planar CT can reveal hardening of the bony labyrinth wall.
