Is this condition phimosis? Is surgery needed as soon as possible?

Patient's question:

Patient is male, 17 years old
He has never really understood this kind of knowledge before, but he happened to see it today and realized that he at least has phimosis
Usually, the glans is completely covered, only a little bit is exposed after erection, and it’s difficult to retract the glans after erection
Instead, he can bear the pain and retract the glans during normal times, and after retracting, the glans just feels a bit numb. However, the area where the glans connects to the foreskin feels very painful, and it looks like the foreskin ring is quite narrower than the glans………………
He can retract it back…………
What kind of problem is this…………
Does he need surgery as soon as possible?

Doctor's answer:

Hypospadias can be considered for surgical treatment. Hypospadias refers to a condition where the penis, in a relaxed state, is covered by the foreskin entirely. If the foreskin not only covers the entire glans but also has a narrow opening and cannot be retracted, this condition is called phimosis. Hypospadias, although it does not affect urination, can harbor dirt and debris in the foreskin, especially as the foreskin can secrete a foul-smelling white discharge called smegma. Long-term irritation of the glans, particularly the corona, by smegma can easily lead to balanitis and penile inflammation. Bacterial infections from the foreskin cavity can also cause cystitis, pyelonephritis, and prostatitis.
