Generally, active treatment is required in two situations: causing male infertility, and secondly, varicocele can cause discomfort such as a sense of fullness in the perineum; however, it is clear before surgery that there is a risk of no improvement or only slight improvement in both cases postoperatively; additionally, there is a possibility of recurrence after surgery. For those with mild clinical symptoms or normal semen quality, observation may be sufficient.
