Does orchitis really cause infertility?

Patient's question:

Got married just this year. I heard that epididymitis can cause infertility, is that true? Please ask the expert for a detailed answer, thank you

Doctor's answer:

Epididymitis primarily damages the male reproductive system through inflammatory stenosis of the seminal tract, causing obstruction. Epididymitis not only leads to obstruction at the connection between the epididymal duct and the vas deferens but also causes vas deferens obstruction. To assess the degree of obstruction, a routine semen analysis should be performed. If epididymitis results in azoospermia without a clear obstruction site, retrograde vasography is recommended.
Currently, there are no specific drugs for treating epididymitis in traditional Chinese or Western medicine. In China, a few doctors who have studied abroad have introduced relatively effective treatments for epididymitis, such as reanastomosis surgery, with some success cases, though the success rate remains low. Additionally, Chinese medicine has no definitive surgical approach for inflammatory obstruction of the vas deferens. For surgical treatment involving the epididymal duct and vas deferens segment, the recommended method is microscopic longitudinal double-needle overlay anastomosis of the epididymis and vas deferens. However, no feasible surgical therapy has been identified for vas deferens obstruction (vas deferens anastomosis is only effective for tubal ligation).
Compared to these options, the best approach currently is to use traditional Chinese medicine for treatment. It is recommended to try "Miao Shan Xiao Yong Tang." The reason "Miao Shan Xiao Yong Tang" stands out among various treatments is due to its fundamental effectiveness. It is advisable to seek timely treatment, as the risk of infertility remains significant otherwise.
