What are the factors or causes of infertility?

Patient's question:

I have been unable to get pregnant. I have been married for two years, but there has been no change. What are the factors or causes of infertility?

Doctor's answer:

1. Vaginal factors: Congenital factors such as vaginal atresia or vaginal septum can cause sexual intercourse difficulties or barriers, thereby affecting sperm entry into the female reproductive tract. Vaginal inflammation alters the biochemical environment of the vagina, reducing sperm motility and survival, which in turn impacts fertility chances.
2. Cervical factors: Conditions such as cervical stenosis, polyps, tumors, or adhesions can impede sperm passage. Cervical erosion, with its inflammatory secretions, has a spermicidal effect. The presence of antisperm antibodies in cervical mucus is unfavorable for sperm penetration through the cervical canal or may completely render sperm non-motile. Notably, cervical adhesions resulting from induced abortion in unmarried pregnant women, especially those with repeated abortions, are a severe complication that can block sperm from entering the cervical opening.
