Semen does not liquefy, low motility

Patient's question:

Western medicine tests indicated prostatitis, but the medication had no significant effect. Later, I consulted a traditional Chinese medicine (TCM) practitioner, who said that poor sleep quality, poor gastrointestinal health, and physical weakness were causing poor sperm quality. Now, after taking TCM medicine for a month, the liquefaction time is still greater than one hour. The follow-up results showed some improvement but also some deterioration.
The total sperm count has increased more than twofold, but it's unclear whether this is good or bad.

Doctor's answer:

Ejaculate does not liquefy due to a lack of secretion or destruction of proteolytic enzymes, leading to an imbalance in the enzyme system. This results in some patients' semen remaining unliquefied even after being placed for 1 hour, or liquefying incompletely with excessively high viscosity. In such cases, sperm cannot migrate, naturally causing infertility. This condition is associated with infections in accessory glands that impair the secretion of proteolytic enzymes, fibrinolytic enzymes, and semen liquefaction factors, preventing the liquefaction of semen. It is advisable to visit a hospital early for tests such as semen biochemistry to identify the specific cause and provide effective treatment targeting the root problem.
