What happens if varicocele is left untreated for a long time?

Patient's question:

Vessels in the scrotum became thickened and clustered together. I had a B done at the hospital, and I remember the doctor said it was varicose veins, which should be varicocele, right??? At that time, I ran every day, and I ran quite hard each day. After running for a week, one night after sleeping, I felt itchy in the scrotum, and later I found a small soft lump inside. However, the itching didn't last for long, and it basically stopped after a few days.

Doctor's answer:

Analysis of the condition:
Hello, varicocele can be classified into three levels: mild, moderate, and severe.
Suggestions:
The mildest form is when varicose veins are not visible after standing and holding breath but can be felt as a vein cluster, which disappears quickly when lying down. The moderate form is when varicose veins are both visible and palpable as a vein cluster, and the lump also gradually disappears after lying down. The most severe form is when varicose veins are clearly visible to the naked eye and the scrotum feels distinctly like a worm-shaped vein cluster, even if it disappears slowly when lying down. This is classified as severe.
If a varicocele patient only experiences mild discomfort, medication control or scrotal support can be adopted. If the pain is severe or there are concerns about infertility, high ligation surgery should be performed. Asymptomatic mild varicoceles do not require treatment.
