How do you always fail to ejaculate after sex?

Patient's question:

How do you always fail to ejaculate after sex?

Doctor's answer:

Hello!
Ejaculatory dysfunction refers to a condition where the penis can become erect and hard during sexual intercourse but fails to ejaculate or cannot ejaculate inside the female vagina, resulting in an inability to reach sexual climax. The erect penis gradually softens and returns to normal after a period of time. It is one of the male sexual dysfunctions. Primary ejaculatory dysfunction means that the erect penis has never been able to ejaculate inside the vagina. If there was previously the ability to ejaculate during intercourse but this ability is now lost, it is referred to as secondary ejaculatory dysfunction. Clinically, primary ejaculatory dysfunction is more common than secondary dysfunction.
The organic causes of ejaculatory dysfunction are mostly congenital anatomical abnormalities of the reproductive system, spinal cord damage, lumbar sympathetic ganglion injury, or the use of drugs that affect sympathetic nerve tone (e.g., guanethidine, thiazide-like drugs). Most cases of ejaculatory dysfunction remain psychological in nature. This may stem from severe religious or feudalist abstinence education received before development, leading to the incorrect belief that sexual activity is sinful, masturbation is evil, or harboring hostility toward the female partner. Other causes include specific psychosocial traumas (e.g., the female partner engaging in extramarital affairs or being raped) or fear of pregnancy, all of which create significant psychological burdens.
In some cases, male ejaculatory dysfunction is due to a lack of sexual knowledge, or "sexual blindness." During intercourse, the penis may not move inside the vagina or may move with small amplitude and low frequency, failing to reach climax and thus not ejaculating. This type of "sexual blindness" can be corrected with proper explanation. Strictly speaking, this condition should not be termed "ejaculatory dysfunction" but rather a lack of understanding of the proper technique.
