How to treat epididymal cyst on the right side

Patient's question:

Right epididymal cyst, approximately 1.8 cm, asymptomatic, no treatment history

Doctor's answer:

Hello, regarding epididymal cysts, also known as seminal cysts, they are most common in the age group of 20 to 40 years. The cause of epididymal cysts may be due to obstruction of the vas deferens leading to the accumulation of semen. The common location for epididymal cysts is the head of the epididymis, while the body and tail are rarely affected. They originate from the epithelial cells of the efferent ductules of the testicular meshwork. The diameter is usually several millimeters to several centimeters, and they can be a single cavity or multipleated cavities, but single cysts are more common. The cyst fluid often contains sperm. Clinical manifestations typically include a feeling of heaviness or swelling in the scrotum, without any specific discomfort, and the condition progresses slowly. A round or oval mass can be palpated at the head of the epididymis, with a smooth surface, no tenderness, cystic consistency, and clear boundaries with surrounding tissues, without adhesion. Small cysts without symptoms can be observed without treatment, and they generally do not affect fertility. For more severe symptoms or larger cysts in married individuals with children, surgical removal may be performed. Surgery may damage the epididymal ducts and affect fertility, so unmarried patients should be cautious.
