Doctor's answer:
Balanoposthitis is caused by local uncleanliness and the long-term accumulation of smegma, leading to secondary infection. It commonly occurs in cases of phimosis or excessive prepuce. The main symptoms include local pain, which worsens during urination, and the possibility of purulent discharge. Examination reveals congestion and edema of the preputial area, with purulent discharge when the prepuce is squeezed. In severe cases, skin erosion and ulcers may develop. Treatment involves selecting appropriate antibiotics, cleaning the preputial area with a 1:5000 potassium permanganate solution, and applying antibiotic ointment. After the inflammation subsides, individuals with phimosis or excessive prepuce may undergo circumcision to prevent recurrent balanoposthitis. Many patients develop redness or small papules on the glans penis and suspect they may have a sexually transmitted disease.
