How to deal with varicocele in a 14-year-old child

Patient's question:

Previously, a hard lump was found in the left testicle. After checking at the hospital, it was confirmed to be varicocele. I don't know the best way to treat it.
Past treatment history and effectiveness: The doctor said it was the mildest form and that surgery could be postponed for now. Annual check-ups for sperm quality can be done, but the child is too young to undergo this test.
Help needed: Please advise the doctor on the best course of treatment.

Doctor's answer:

Analysis of the condition: Varicocele is caused by venous stasis in the internal spermatic vein, leading to the dilatation and tortuosity of the venous plexus. It is more common in young and middle-aged adults, often occurring on the left side. Mild cases may be asymptomatic, while severe cases may present with a dull ache in the affected scrotum during prolonged standing, heavy lifting, or exertion, which improves with rest in a supine position. Physical examination may reveal a relaxed scrotum on the affected side, with a palpable worm-like mass.
Advice and recommendations: The patient is 14 years old and has not yet reached marriageable age. It is recommended to postpone surgery for the time being. After marriage, regular follow-up of sperm quality is advised. Currently, conservative treatment can be initiated, such as wearing elastic pants or a scrotal support band. If sperm quality declines or the dull ache becomes severe, surgical intervention may be considered.
