Doctor's answer:

Hello: This is scrotal eczema, and its cause is relatively complex, involving both internal and external factors. People with an allergic constitution, those who are under long-term mental stress or experience significant emotional fluctuations, are prone to this condition. Additionally, individuals with certain diseases, such as chronic digestive system disorders, gastrointestinal dysfunction, endocrine disorders, or metabolic disturbances, are also susceptible to this condition under the influence of external factors.
External factors include:
① A humid living or working environment with high air humidity;
② External stimuli, such as cold or heat, excessive sweating, or over scratching;
③ Tight underwear or friction from foreign objects, as well as wearing synthetic underwear, can all trigger scrotal eczema.
Generally, antihistamines can be used. Commonly prescribed ones include:
- Cyproheptadine 2mg, taken three times daily;
- Chlorpheniramine 4mg, taken three times daily;
- Astemizole 25mg, taken three times daily;
- Loratadine 10mg, taken once daily.
Topical medications primarily involve corticosteroid creams, such as Hydrocortisone cream, Filgrastim cream, and Eczema cream.
Scrotal eczema is a stubborn and difficult-to-treat condition. Prevention should include the following measures:
1) Wear loose and comfortable underwear, preferably made of pure cotton, and avoid tight underwear. Change and wash underwear promptly, especially after exercise, to keep the area clean.
2) In terms of diet, consume more fresh vegetables and fruits, and avoid or limit the intake of greasy or spicy foods.
3) If scrotal itching occurs, seek active treatment and avoid excessive scratching or washing with hot water, especially avoiding the use of soapy water for washing.
