What should I do if I can't climax during sex with my partner?

Patient's question:

For a while now, sometimes after doing it, I can't, and I haven't gone to the hospital.

Doctor's answer:

Ejaculation failure is a type of male sexual dysfunction, more severe than impotence or premature ejaculation, and requires greater patience in treatment. If ejaculation failure occurs, it is unnecessary to panic excessively, as this may worsen psychological burdens. It is important to seek medical attention promptly at a reputable and professional hospital to first rule out organic causes, such as anatomical abnormalities in the urogenital system, spinal cord disorders, or the use of drugs that affect sympathetic nerve function (e.g., guanethidine).
There are three distinct types of ejaculation failure:
① The male experiences sexual arousal and reaches orgasm, but there is no ejaculation or semen leakage. This is due to the failure of the sacral spinal cord's ejaculation center.
② The male experiences sexual arousal and attempts to ejaculate, but no semen is released. This is caused by dyscoordination of the ejaculation center, often seen in cases of autonomic nervous system dysfunction resulting from diabetes.
③ During sexual activity, the male does not experience sexual arousal, ejaculation, or semen leakage. This is due to dysfunction of the cerebral cortex, which exerts excessive inhibition on lower sexual centers.
Organic causes of ejaculation failure are rare. The majority of patients with ejaculation failure are influenced by psychological factors. The main reasons are as follows:
① A lack of a scientific understanding of sexual issues, influenced by feudalism, superstition, or religion.
② Exaggerating the harmfulness of masturbation, leading to psychological burdens; or excessive masturbation, as the stimulation to the genitalia during masturbation is stronger than during intercourse, causing some individuals to become dependent on masturbation and raising the threshold for sexual arousal, so that genital contact fails to achieve sufficient excitement.
③ Disharmony in the marital relationship.
④ Fear of pregnancy in the female partner. Additionally, many people do experience ejaculation failure due to excessive sexual activity, such as multiple sexual encounters in one night. Such patients are advised to abstain from sexual activity for one month before resuming intercourse.
