Patient's question:
A five-year-old child in my family accidentally suffered burns. I would like to ask how the fluid calculation for burn treatment is determined? Does the amount of fluid vary depending on the duration of the burn?Doctor's answer:
Fluid replacement in the first 24 hours post-injury: For every kilogram of body weight and 1% of burn area, the patient requires 1.5 ml of fluid (1.8 ml for children, 2 ml for infants). Total fluid replacement in the first 24 hours = Weight (kg) × Burn area (%) × 1.5 + 2000 ml of daily physiological fluid needs. The ratio of crystalloid solution to colloid solution is 2:1. For extensive and deep burns, the ratio is 1:1.
Fluid replacement in the second 24 hours post-injury is half of the first 24 hours, with the daily physiological fluid needs remaining unchanged.
