Features: "Differential Diagnosis of TCM Symptoms" is an important part of TCM differential diagnosis. "Symptom differential diagnosis" refers to the analysis of "symptoms" using the basic theories and dialectical methods of TCM. It involves analyzing the characteristics of the same symptom when it appears in different "syndromes" and identifying which syndromes the same symptom may be associated with. The content discussed in "symptom differential diagnosis" focuses on the differentiation between different syndromes that share the same primary symptom. The issues addressed are those where the primary symptom is the same, but the syndromes differ, rather than the differentiation between different "symptoms," which must first be clarified. For example, the symptom of "tongue numbness" can manifest in syndromes such as "blood deficiency tongue numbness," "liver wind tongue numbness," and "phlegm obstruction tongue numbness." While the same symptom of tongue numbness exists, the syndromes differ, and the reason for this difference lies in the different etiology and pathogenesis of the syndromes. If further research is conducted on the syndromes, it will also be found that their clinical "complementary symptoms" have different characteristics. For instance:
- The characteristics of "blood deficiency tongue numbness" are a pale and numb tongue, with complementary symptoms such as pale and yellow complexion, palpitations, shortness of breath, and weak, thready pulse.
- The characteristics of "liver wind tongue numbness" are numbness and rigidity of the tongue, with complementary symptoms such as difficulty in speech or sudden collapse, and wiry, thready, and rapid pulse.
- The characteristics of "phlegm obstruction tongue numbness" are numbness of the tongue with thick, greasy coating, with complementary symptoms such as dizziness and slippery pulse.
In clinical differentiation, it is impossible to distinguish between different syndromes without complementary symptoms. Therefore, when analyzing the etiology and pathogenesis of a primary symptom, it is essential to consider the complementary symptoms that coexist with the primary symptom comprehensively and not analyze the symptom and pathogenesis in isolation. Without complementary symptoms, it is impossible to understand the primary symptom, just as a thing cannot be compared if it is disconnected from its surrounding context. This makes differentiation impossible.
TCM differential diagnosis of symptoms precisely summarizes the patterns of the same primary symptom appearing in different syndromes, reveals their etiology and pathogenesis, and especially highlights key points through external manifestations (complementary symptoms), making it easier for clinical practitioners to grasp. This differentiation pattern between syndromes with the same primary symptom is the distinctive feature of TCM differential diagnosis of symptoms.
