Patient is 40 years old, discovered in 2010 to have varicocele leading to asthenozoospermia

If it can be confirmed that the low sperm count is caused by varicocele, then surgical treatment is the only option. As for whether surgical treatment or artificial insemination should be chosen, my suggestion is that you can have your partner undergo a fallopian tube first. If it is unobstructed, I believe that if the semen quality is acceptable, considering your age, I recommend artificial insemination. If it is obstructed, I suggest you proceed directly with IVF.
I do not recommend surgical treatment for you for two reasons:
1. Surgical improvement in sperm motility is not guaranteed to be 60%-70%, with the maximum improvement being only 30%, and the improvement takes 3-6 months.
2. The issue of time is very important, as the older you are, the lower the success rates for both IVF and artificial insemination.
