How is hematospermia caused?

Patient's question:

A while ago, my husband was under a lot of stress. Later, when we had sex, he noticed that the semen he released had a bloody sensation. At the time, he thought it was due to a "hot constitution" and didn't pay much attention. However, it happened several times during intercourse, and my husband has very healthy living habits.

Doctor's answer:

Hello! Most cases are caused by seminal vesiculitis. Due to the thin wall of the seminal vesicles, if inflammation and congestion occur, or if lesions such as stones develop, the blood-filled wall of the vesicles can easily bleed. The secretion of the seminal vesicles is one of the main components of semen. When the vesicles bleed, their secretion becomes stained with blood. If this secretion is expelled from the body along with semen, hematospermia occurs. Chronic prostatitis can also cause hematospermia, with a mechanism similar to seminal vesiculitis. Since prostatic fluid is another important component of semen, inflammation and congestion of the prostate lead to exudation; frequent sexual activity can cause capillary rupture; and the strong contraction and relaxation of the prostate during ejaculation can also cause the prostatic fluid to be mixed with blood, which in turn contaminates the semen with blood stains. Similar to chronic prostatitis, benign prostatic hyperplasia can also lead to hematospermia. It is worth noting that hematospermia caused by chronic prostatitis or seminal vesiculitis is often related to excessive sexual activity or rough movements.
