Should a child with excessive foreskin have surgery?

Hypospadias in children is a normal phenomenon. As they grow older and their penis develops, the foreskin can retract on its own, exposing the glans. There is no need for premature surgery. If there is recurrent infection, adhesion between the glans and foreskin, accumulation of smegma in the foreskin cavity, or narrowing of the foreskin opening, the foreskin opening can be dilated with forceps under local anesthesia, the adhesion separated, the smegma cleared, and erythromycin ointment applied to the glans. Afterward, daily cleaning of the foreskin should be practiced to prevent re-adhesion.
