II degree: Varicose veins are palpable in an upright position but appear normal externally.
III degree: Varicose veins are visible on the surface of the scrotum, and a soft, worm-like mass can be palpated.
Subclinical varicocele: The diameter of the spermatic vein is greater than 2 mm.
Clinical varicocele: The diameter of the spermatic vein is greater than 5 mm.
Whether surgery is needed depends on:
1. Impact on fertility (infertility or abnormal semen parameters);
2. Symptoms (scrotal heaviness, swelling, or pain).
Surgery: High ligation of the internal spermatic vein.
