When the foreskin is too long or phimotic, this substance can accumulate as smegma, irritating the foreskin and glans, leading to balanoposthitis. The basic principle of Western medicine treatment is anti-inflammatory and antibacterial. Generally, it involves both topical and systemic treatment. For acute superficial balanoposthitis and annular erosive balanoposthitis, erythromycin and tetracycline are the first-line treatments. For trichomoniasis balanoposthitis, metronidazole is the preferred treatment. For candidal balanoposthitis caused by Candida, griseofulvin or itraconazole is commonly used.
