Patient's question:
Doctor,I had sex with my boyfriend yesterday, and he said his semen had small clumps. I don't know what this could be, and whether it indicates any abnormal condition. What illnesses could cause this? What precautions should I take? Thank you!
Doctor's answer:
Hello: Your symptoms may be mild sperm nonliquefaction. Methods for treating sperm nonliquefaction include the following:
(1) Completely treat the primary diseases such as prostatitis and seminal vesiculitis to restore their normal function.
(2) Use physiological saline containing 4% α-amylase to rinse the vagina before sexual intercourse, or inject 1 milliliter into the vagina after intercourse. Alternatively, 50 milligrams of the enzyme can be mixed with cocoa butter to form a 3-centimeter-long suppository, which is inserted into the vagina before intercourse. This method was discovered because someone observed that saliva has the effect of liquefying semen and was inspired by it. α-amylase not only promotes the liquefaction of semen but also affects vaginal or cervical secretions containing glycogen, and can also serve as an energy source for sperm activity. Therefore, this method has been widely used.
(3) Add normal human seminal plasma to the semen to promote liquefaction. However, it is first necessary to ensure that this seminal plasma does not contain antisperm antibodies, is free of infection, and has been pre-treated with high-speed centrifugation to remove precipitates and undergo three cycles of freezing and thawing.
(4) Ejaculate the semen into a container filled with culture medium, repeatedly aspirate with a syringe equipped with 18 or 19 gauge needles until the semen becomes thin, then perform two rounds of centrifugation and washing, and finally suspend the sperm in a certain amount of culture medium for artificial insemination.
(5) If the semen has a high viscosity, the divided ejaculation method can be used: the first three emissions of semen are left in the vagina, the penis is immediately withdrawn, and the remaining semen is expelled outside. This is because the posterior semen mainly contains seminal vesicle secretion, which has a higher viscosity.
