Is it a disease if the testicles have nodules?

Patient's question:

Patient Gender: 28 years old
Patient Age: Male
Detailed Condition and Purpose of Consultation: One year ago, I noticed a pea-sized nodule at the lower end of the right testicle. It doesn’t hurt or swell when touched. Sometimes, after exertion, I feel a sense of heaviness and fullness in the testicle. A few days ago, lifting heavy objects made my right side feel extremely swollen. I also have mild prostatitis. Could the nodule and heaviness in the testicle be caused by prostatitis, or is there another reason? Could it be testicular cancer? If I suspect testicular cancer, what tests should I undergo at the hospital? Should I go to the surgical department or the urology department?
Additional Tests: None

Doctor's answer:

This seems to be orchitis. In adults, it is often caused by the direct spread of epididymitis to the testicles or by hematogenous infection. Clinically, it is commonly referred to as epididymo-orchitis. The diagnostic criteria for epididymo-orchitis include:
1. High fever, chills, and testicular pain.
2. Local scrotal redness and swelling, with testicular enlargement.
3. Elevated white blood cell count. It is recommended to have an ultrasound for confirmation.
Treatment includes:
1. Antibacterial and anti-inflammatory therapy. Early use of sufficient, broad-spectrum antibiotics, such as a combination of cephalosporins and quinolones.
2. Bed rest, elevating the scrotum, local heat application, and physical therapy. A 1% lidocaine spermatic cord block injection can help reduce pain.
3. If an abscess forms, it must be drained surgically.
Wishing you a speedy recovery.
