How to treat sexual function problems

Patient's question:

During sexual activity, sensitive and rapid erection, with sexual activity time always lasting between 2-4 minutes

Doctor's answer:

Before treatment begins, it should be possible to understand the cause as much as possible, help the patient recognize their condition and formulate appropriate treatment measures. If low libido is related to environmental or family factors, the problem should be pointed out to the patient, and suitable solutions should be proposed. For those with significant abnormal experiences during childhood or adolescence, psychoanalytic therapy can be tried to identify potential underlying reasons or traumatic factors that suppress libido, combined with sexual education for targeted treatment.
A harmonious marital relationship, mutual understanding, mutual respect, and open communication are necessary conditions for improving low libido. Improving the sexual relationship between spouses should be a key focus of treatment. The treatment plan should be tailored to specific circumstances and needs, and before formulating the plan, the types of sexual behavior between the couple should be pre-assessed. There are three types of thoughts about the sexual relationship that may mutually influence and potentially harm the couple:
Guidelines:
The first thought is that sexual satisfaction and pleasure depend on initial sexual interest. This idea confuses sexual contact and sexual arousal. If objective reasons temporarily reduce interest in sexual activity, the person may believe they cannot participate in sexual activity in the future.
The second thought is that sexual activity will always lead to intercourse and orgasm.
The third thought is that habitual sexual routines cannot be changed.
These three thoughts are often directly or indirectly transmitted from one spouse to the other, leading to outdated and involuntary sexual behavior. To address these misconceptions, treatment should focus on the following aspects. First, discuss these thoughts directly and repeatedly with the patient couple, explaining their relationship to low libido, encouraging them to alleviate their concerns, and helping them identify factors affecting sexual interest. Second, guide the couple in specific methods, such as sensate focus exercises, with the emphasis on gradual progress.
Lifestyle Care:
Pay attention to verbal and non-verbal communication. Do not treat sexual arousal and intercourse as the goal. Encourage sexual activity without frustration or discomfort to reinforce these views and boost confidence. For patients with sexual dysfunction such as erectile dysfunction or premature ejaculation, symptomatic treatment should be provided. Often, after sexual function improves, libido also returns to normal. Short-term use of male sex hormone therapy, such as oral methyltestosterone or intramuscular testosterone enanthate, may also be effective.
