What are the reasons for painful urination?

Patient's question:

I only recently noticed that my lower back has been a bit painful, and occasionally there's a symptom of white liquid after urination (this has been going on for a while). Can anyone explain this, and provide some treatment suggestions?

Doctor's answer:

If you are a woman, the possibility of vaginitis is high. Generally, vaginitis can be classified into several types:
1. Candidal Vaginitis: It includes symptoms such as itching of the external genitalia and vaginal area, cheese-like or curd-like vaginal discharge, and the presence of white pseudomembranes in the vagina. A positive fungal test result indicates this type of vaginitis.
2. Bacterial Vaginosis:
① Non-purulent grayish-white viscous vaginal discharge;
② Vaginal discharge with a fishy odor, and a positive amine test;
③ Elevated vaginal discharge pH value (5.0–5.5);
④ The presence of clue cells in the discharge.
3. Mold Vaginitis is caused by an infection with Candida albicans, a type of mold. It is more common in pregnant women and diabetic patients.
The main symptoms of mold vaginitis include burning pain in the external genitalia, increased vaginal discharge, and the emission of curd-like or bean curd-like vaginal. Therefore, a smear test of vaginal discharge can be performed to identify Candida albicans, confirming the diagnosis.
Oral medications such as clotrimazole, nystatin, and ketoconazole are effective. Vaginal suppositories can also be used. Additionally, it is recommended to rinse the vagina and clean the external genitalia 1–2 times daily with an alkaline solution, such as a 2%–4% baking soda solution.
