How to handle frequent hematospermia

Patient's question:

I often have hematospermia. The first ejaculation during coitus is normal in color, but the second or third ejaculation turns red. It gets better after some time, but it reappears if I have multiple coitus in one night. This has never happened before; it only started this year. My blood tests and urine tests are normal, and my ultrasound of the prostate, testicles, and seminal vesicles is also normal. Why is this happening?

Doctor's answer:

Hello, hematospermia is one of the diseases of the male reproductive system, with its main symptom being the emission of red semen during sexual intercourse. It is most commonly seen in modern medicine as seminal vesiculitis and is relatively rare in clinical practice. This condition is often complicated with prostatitis, with its infection pathways primarily being direct extension from urethral and prostatic infections; the second is lymphatic infection and hematogenous infection.
Advice: If hematospermia occurs only occasionally and no specific changes are found upon examination, it may be due to the rupture of tiny blood vessels in certain tissues during sexual intercourse caused by sudden congestion and mechanical trauma. In such cases of transient hematospermia, there is no need to panic. Simply abstaining from sexual activity for one or two weeks will allow for complete recovery. On the other hand, bleeding caused by inflammation tends to fluctuate in severity but does not last long. It is recommended to seek medical attention as soon as possible to avoid delaying the condition.
