When the foreskin is retracted, the inner surface of the foreskin and the glans appear congested, eroded, with exudate, and even bleeding. Secondary infection may lead to small ulcers and foul-smelling, creamy purulent discharge. Friction with underwear causes pain, and patients often experience difficulty in movement. It may be accompanied by swollen and tender inguinal lymph nodes.
