It is most commonly seen in modern medicine as seminal vesiculitis and is relatively rare in clinical practice. This condition is often complicated with prostatitis, with its infection pathways primarily being direct extension from urethral and prostatic infections; is lymphatic infection and hematogenous infection. Due to bacterial invasion and inflammatory stimulation, the seminal vesicles become congested. During sexual intercourse, smooth muscle and blood vessel contraction lead to the leakage of a large number of red blood cells and pus cells into the semen. In traditional Chinese medicine, hematospermia is often attributed to insufficient kidney yin, excessive ministerial fire forcing blood to flow uncontrollably; or excessive sexual activity causing damage to blood vessels, leading to blood mixing with semen; or due to damp-heat accumulation that irritates the seminal vesicles, causing blood to flow uncontrollably due to heat.
