Most patients with prostatic calculi are asymptomatic. The stones can remain in the prostate for over ten years without causing discomfort. If the stones are larger, symptoms may include frequent urination, hematuria, difficulty urinating, and pain. In some cases, additional symptoms such as low libido, hematospermia, or impotence may occur. For deep-seated prostatic calculi and those prone to formation, transperineal total prostatectomy can be performed to remove the prostate and the stones. If necessary, bilateral seminal vesicles may also be resected. In cases of large, easily formed stones accompanied by benign prostatic hyperplasia, suprapubic prostate and stone enucleation is recommended.
