What causes hematospermia in men

Patient's question:

I've been working too long recently, with very little time to rest. I only get 3 hours of sleep a day, and it's making my head feel dizzy and heavy. I can't take it anymore. It's gotten to the point where even when I'm intimate with my wife, the semen I release has a bit of blood in it.

Doctor's answer:

Hello,
I. Hematospermia is mostly caused by bacterial infections, but it can also result from unrelieved sexual tension, long-term abstinence, or excessive sexual activity leading to organ congestion. The symptoms of hematospermia, similar to seminal vesiculitis, primarily manifest as hematospermia during sexual intercourse, accompanied by mild perineal pain, reduced libido, painful ejaculation, premature ejaculation, or a sense of heaviness and distension.
II. Congenital abnormalities. Seminal vesicle cysts usually show no obvious symptoms. However, when secondary seminal vesiculitis occurs, hematospermia may accompany, leading to recurrent episodes. If the cysts become excessively large, they may compress the bladder or urethra, causing urinary difficulties.
Normal semen consists of sperm and seminal plasma. Seminal plasma primarily originates from the seminal vesicles and prostate, with 60% derived from the seminal vesicles and 30% from the prostate. The remaining 10% is secreted by other tissues such as the epididymis, bulbourethral glands, and ampulla of the vas deferens. Therefore, when the seminal vesicles, prostate, or other organs involved in semen production and transport develop lesions and bleed, hematospermia may occur.
