Complete phimosis, where the foreskin covers the glans and is difficult to retract for cleaning, is usually caused by a tight foreskin ring with a diameter smaller than the glans. This prevents the foreskin from being retracted below the glans. If excessive force is applied, the foreskin ring may get stuck in the penile body, compressing venous blood flow, leading to swelling, pain at the tip of the penis, and even difficulty urinating. It must be surgically removed.
