① For obstructive azoospermia, various surgical procedures can be performed depending on the specific circumstances, such as vasovasostomy, epididymovasostomy, artificial vesiculoposthitis, epididymal sperm retrieval, and transurethral incision.
② For non-obstructive azoospermia, if the cause is endocrine abnormalities (e.g., hypogonadotropic hypogonadism), treatments such as gonadotropin-releasing hormone (GnRH) and gonadotropin therapy, along with testosterone replacement, may be administered. If the cause is a pituitary tumor, surgical resection is recommended. For pituitary microadenomas or idiopathic hyperprolactinemia, bromocriptine can be used. In cases of primary testicular failure accompanied by varicocele, varicocelectomy should be performed.
If a male has congenital absence of testes, severe testicular hypoplasia, secondary testicular atrophy, or azoospermia confirmed by testicular biopsy, spermatogenesis is generally not recoverable through conventional treatments.
