Pain and swelling in the inner foreskin and scrotum, what is the cause?

Patient's question:

The inner foreskin has started to swell, resembling a thick lump (or like a blister). It's slightly itchy and painful, then turns white and forms scabs. Two days later, the scrotum and the area connecting to the anus have patches of hard swelling with small bumps. It's slightly itchy, and the burning pain is quite noticeable. (I ate a lot of spicy food before this outbreak, but I don't know if it's related.) Have I had any unsafe sex?

Doctor's answer:

Penile swelling: First, it could be caused by inflammation, which is known as balanitis. Second, it might be due to the foreskin not being retracted promptly after sexual intercourse, leading to phimosis and subsequent swelling. For foreskin inflammation, it is recommended to retract the foreskin daily, clean it with clean water, and take anti-inflammatory medication. Generally, it can return to normal within 3-5 days. If the swelling is caused by phimosis and edema, it is advisable to apply petroleum jelly for sufficient lubrication and then retract the trapped foreskin. If you can handle it yourself, it can be completed within 1-2 minutes. If you cannot manage it, you must go to the emergency room for phimosis and edema repositioning. Penile swelling is usually not severe, and early detection and treatment lead to a quick recovery.
