What is the difference between nocturnal emission and spermatorrhea?

Patient's question:

What is the difference between nocturnal emission and involuntary ejaculation? I recently experienced a sudden recurrence of morning erections with pre-ejaculate fluid on my underwear. However, I don't know if I had a dream that night. When I searched online, some said it was nocturnal emission, while others said it was involuntary ejaculation.

Doctor's answer:

I noticed seminal fluid residue on the underwear. If it happens occasionally, it might be due to nocturnal emissions. Nocturnal emissions occur because mature males constantly produce new sperm in their bodies. When the sperm accumulate to a certain extent, nocturnal emissions may occur, also known as spontaneous ejaculation. This is physiological and happens occasionally each month. If it is pathological, meaning it occurs frequently, even daily, it is called spermatorrhea.
