What are the symptoms of impotence?

Patient's question:

Recently, when having sexual intercourse with my girlfriend, sometimes during the act, the penis remains fully erect, but there's no sensation during ejaculation. At other times, it's difficult to get erect. I'm not sure if this is premature ejaculation! Seeking advice, please help me understand this?

Doctor's answer:

The initial manifestations of premature ejaculation in men are primarily divided into two categories:
(1) Psychogenic premature ejaculation initial manifestations: The onset is relatively sudden, with spontaneous erections of the penis. During deep sleep at night or upon waking, sexual activity or erotic imagination can induce erections. However, when attempting to engage in sexual intercourse, the penis fails to become erect. Alternatively, the penis may become rigid upon initial contact with the female body but becomes soft when attempting penetration. Additionally, mental symptoms such as anxiety, worry, depression, and fatigue may accompany the condition. Some cases may also involve premature ejaculation or anejaculation during intercourse.
(2) Organic premature ejaculation initial manifestations: The main symptom is the inability to achieve an erection under any circumstances. The onset is typically slower and tends to worsen progressively.
