Is there a problem with the testicles being of uneven height?

Patient's question:

Doctor, hello. I have been in a relationship with my boyfriend for about half a year. Usually, I notice that his body is very healthy, he has a lot of energy when working, and he rarely catches colds. Last night, we had sex together, and I suddenly noticed that his testicles were of uneven height. Could you please advise me? Is it a problem if the testicles are of uneven height?

Doctor's answer:

The testicles are not of equal height normally; they are usually higher on the left and lower on the right, which is a normal physiological manifestation. If there is a split in the unequal size of the testicles, such as excessive swelling on one side with symptoms of redness, heat, and pain, it is commonly associated with orchitis, epididymitis, inguinal direct hernia, or inguinal. Further examination through a detailed testicular ultrasound is necessary to confirm the diagnosis. If it is indicated to be a hernia, surgical treatment should be combined.
